A Consent to Contact (CTC) is required when bringing on a new client. This document gives the company case worker permission to contact the client using the client chosen methods of contact (email, voice call, or text message). Will provide info when signing. This info will update the clients member profile.

Signatures will capture date signed. A document is required for any adults in the case. And a document is required to be filled out if the client is a minor. The document must be signed by the parent or legally responsible party
